Applicants must be in line 30 minutes prior to end time.


What You Need to Bring

  • Proof of Citizenship – must be an original U.S. Birth Certificate or Citizenship Certificate or expired Passport.  No photocopies or birth cards will be accepted.
  • One passport photo. This photo must be a clear, well-lit, 2" x 2" photo with white background.   ***Starting October 1, 2016, you will no longer be allowed to wear glasses when taking your passport photo.  If you are wearing glasses in your picture, we will not be able to accept the application. 
  •  Valid government issued photo id (Florida driver’s license, Florida identification card)
  • When applying for children 15 and under, child must be present as well as both parents or legal guardian and you must have birth certificate.
  • Check or money order for each application processed
  • Passport Application - the DS-11 form can be downloaded from the U.S. Department of State website and brought with you or we can provide you with an application to complete at time of processing.
  • All applicants must be present to submit passport application
  • Please note - all applicants must put their Social Security number on their application when they submit the form for processing. 


THE DS-82 FORM IS NOT ACCEPTED AT THE CITY OF COCONUT CREEK - THE DS-82 FORM IS MAILED OUT BY "YOU", THE APPLICANT.

RENEWAL - U.S. PASSPORT RENEWAL APPLICATION FOR ELIGIBLE INDIVIDUALS (FORM DS-82)
This form may ONLY be used if YES is answered to ALL six questions:

  1. I can submit my most recent U.S. passport book and/or U.S. passport card with this application.
  2. I was at least 16 years old when my most recent U.S. passport book and/or U.S. passport card was issued.
  3. I was issued my most recent U.S. passport book and/or U.S. passport card less than 15 years ago.
  4. My most recent U.S. passport book and/or U.S. passport card for which I am renewing has not been lost, stolen, mutilated, or damaged.
  5. My U.S. passport book and/or U.S. passport card has not been limited from the normal ten (10) year validity period due to passport damage/mutilation, multiple passport thefts/losses, or non-compliance with 22 C.F.R. 51.41 (Please refer to the back pages of your U.S. passport book for endorsement information).
  6. I use the same name as on my recent U.S. passport book and/or U.S. passport card.
    -OR-
    I have had my name changed by marriage or court order and can submit proper documentation to reflect my name change.

If you have answered YES to the questions above you are eligible to use the renewal form DS-82. You can download the DS-82 form from the U.S. Department of State website.

If you answered NO to any of the six statements above, STOP - you cannot use this form.  You must apply using application DS-11 by making a personal appearance before an acceptance passport agent.


Pricing  **PLEASE NOTE-BEGINNING APRIL 2, 2018, THE FEE FOR THE CITY OF COCONUT CREEK WILL CHANGE TO $35 AS PER THE U.S. DEPARTMENT OF STATE FEE SCHEDULE**

 Routine Service (processing time will take 6-8 weeks)
Adults                          $110 to the U.S. Dept. of State – must be check or money order
(age 16 and up)         $35 to the City of Coconut Creek

Child                             $80 to the U.S. Dept of State – must be check or money order
(age 15 and under)    $35 to the City of Coconut Creek

Expedite Service (processing time will take 2-3 weeks)
An additional fee of $60 added to the fee paid to the U.S. Department of State for expedited service
