Right of Entry Form
Creek Requests Home Owners Associations (HOAs), Mobile Home Parks, and
Apartment Complexes to Submit Permit and Declaration Right of Entry Forms for Hurricane Prep
The City of Coconut Creek strongly encourages all homeowner associations, mobile home parks,
and apartment complexes to fill out and submit the “Permit and Declaration Right of Entry,
Hold Harmless and Non-Duplication of Benefits Form” as soon as possible for hurricane preparation.
In order for the City to perform curbside debris removal on privately-owned property, the Form must
be completed by an authorized HOA Board Member, property manager, or property maintenance company
and is maintained on file by the City on an annual basis.
This document will provide authorization for the City to enter private property where operationally
feasible and as determined by the City’s designated Debris Management Coordinator to perform debris
removal on privately-owned property. However, this permit does not represent an obligation upon the
City to perform such work.
The Form can be obtained at City Hall’s reception desk from Monday through Thursday, 7:00 AM – 6:00 PM
